INTRODUCTION TO MALE INFERTILITY:
Male infertility is a well known disease since ages and there are
multiple causes of male infertility. There may be sperm count less than twenty
million per cubic centimeters of ejaculated semen or there may be no sperm in
the semen at all which is called azoospermia in medical terminology.
TYPES OF MALE INFERTILITY:
There are two types of azoospermia, one is due to blockage of
sperm transport from its site of production to the penis. Blockage can occur at
the level of small tortuous tube called epididymis or at the level of more
distally present vas deferens. Trauma, infection, surgery or varicose veins are
among well known causes for interruption of sperm flow in the epididymal tract.
Other type of azoospermia is due to failure of production of
sperms which is again multifactorial in nature. There may be insult to testes
resulting in total failure to produce sperms. Such insult can occur due to
trauma, surgery, infections like orchitis or idiopathic conditions. Mumps orchitis
is a well known entity associated with total failure of sperm production.
HORMONAL CAUSES OF MALE INFERTILITY:
Azoospermia can also be caused by hormonal deficiency at pituitary
gland level where testosterone and luteinizing hormones are synthesized. These hormones
have positive feedback effect in sperm production and low levels of these
hormones cause treatable form of azoospermia.
OBSTRUCTIVE AZOOSPERMIA TREATMENT OPTIONS:
Obstructive azoospermia is treatable in most cases depending on
the cause but production deficiency is usually untreatable in most but not all cases.
Correcting hormonal deficiency brings sperm synthesis to normal or near normal
levels hence improving total sperm count. Intravenous injection of testosterone
analogues is the treatment method of choice for underproduction of
testosterone.
Sometimes testosterone levels are normal in blood but the receptors
where it acts are non responsive. This condition is difficult to treat and the
patient may have to live his whole life with infertility.
ERECTILE DYSFUCTION AND INFERTILITY:
If the problem is with erectile function, causes are multiple.
This may be due to underlying disease like diabetic neuropathy which damages
the splanchnic nerves. In such cases treatment of underlying cause in
undertaken. For diabetic neuropathy, gabapentin or duloxetine are used.
Impotence may be due to psychosocial impacts. In such cases psycho-therapy
is the treatment of choice.
TREATMENT OF ERECTILE DYSFUCTION:
A well known drug for treatment of erectile dysfunctions is
sildenafil. This is vasodilator drug which dilates the arterial channels of
penile musculature thereby improving blood circulation to the venous lakes of
penile muscles. In this way blood pools in the penis causing erection. But there
are some potential side effects of this drug, headache and low blood pressure
to name.
ROLE OF HERBAL MEDICATION IN THE TREATMENT OF MALE INFERTILITY:
There are treatments available for each type of infertility. Herbal
medicines have been used in treating infertility since ages. These products are
natural and have minimum side effects as compared to some other methods of
treatment. Many useful herbal products have been introduced for this purpose
with variable effects.
